The unseeded Saints booked their place in the final after overcoming Anglo Chinese Junior College (ACJC) 3-2 on penalties.
YJC were led with a game-high 19 points from Devin Tan (#10), along with 12 points from compatriot Hua Haowei (#9).
HCI completed the race in 3:31.15 to clinch the silver medal, while Anglo-Chinese School (Independent) bagged the bronze medal with a timing of 3:34.04.
While HCI had the obvious lead throughout the race, the first runners-up were only decided in the final metre.
HCI's Naomi Louise Tan was second in 12.97s while third-placer Jerrine Lim of ACJC finished in 13.20s.
Victoria Junior College finished second in 51.47 seconds, while Raffles Institution claimed the bronze in 52.09 seconds.
Hwa Chong Institution's Ong Sim Hao (#273) settled for the silver medal after a count back, while his teammate David Chung cleared a height of 1.91 metres to complete the podium.
